Cancellation Policies

Cancellation Policy for Cosmetic Appointments

All cosmetic appointments require a 48-hour cancellation notice in order for you to receive a full refund of your deposit. If you cancel your appointment within 24 hours, you will forfeit 50% of your deposit and 50% will be remitted to you. If you cancel or reschedule your appointment with less than a 24-hour notice, you will forfeit 100% of your deposit.

Cancellation Policy for General Dermatology Appointments

We require a 24-hour notice if you cancel or reschedule a general dermatology appointment.  A fee of $50.00 will be charged for appointments not canceled or rescheduled 24 hours in advance.

This notice describes how health information about you, as a patient of this practice, may be used and disclosed, and how you can get access to your individually identifiable health information.

Prescription Refills

Call the office during business hours to request a prescription refill.  We will determine if the prescription may be refilled by phone depending on your condition and the date of your last visit to the practice.  If you have not been seen in our office in the past 6 months, we cannot call in a prescription refill.
